The undersigned, know that running a 5K/Fun Walk is potentially hazardous activity. I am medically able and properly trained for the race. I assume all risks with running with running in the event. Having read this waiver and knowing these facts and in consideration of acceptance of my entry, I, for myself and heirs, executor or anyone entitled to act on my behalf, waive and release the National MPS Society and its employees and any of their representatives and successors from all claims of any nature whatever foreseen or unforeseen known or unknown to my person or property. INFORMATION ABOUT REGISTRATION FEES:
* Register before August 10, 2011 for this discounted registration fee.
Register between August 11 and September 9, 2011 registration fee for 5k walk is $22.00 and 1 mile walk is $18.00.
Same day registration for 5k run is $25.00 and 1 mile walk is $20.00
Those who ran the 1st Annual event will receive a discount on this years registration fees.
